To ensure the delivery of exceptional, comprehensive care, our residents also have access to a variety of medical specialists, including a pharmacist, podiatrist, dentist, ophthalmologist and other visiting health care professionals.

Comprehensive Therapies

Long-term care residents at Manchester Manor may also be eligible to receive to a full range of physical, occupational and speech therapy as well as therapeutic recreation, if needed, which are designed to help regain – or simply maintain – their strength, mobility and balance.

At Manchester Manor, we use advanced technology to improve the environment, to aid our clinicians in treatment and predictive care to accurately monitor and record health concerns, including:

Medline EKG Monitor – Accurately records, stores and shares cardiac rhythms for easy and quick diagnosis and care
Direct Supply Doppler Ultrasound – An important tool for clinicians, this is a non-invasive way to provide live audio of blood flow
PointRight – Supports quality improvements through predictive analytics, with emphasis on reducing unnecessary hospital readmissions and enhancing the quality of care for patients
PointClickCare Post–Acute Electronic Health Record – Software that allows Manchester Manor to maintain a single, consistent repository of patient information and conduct a central hub to manage financial, clinical and administrative functions
RTMS – Software that works in conjunction with our EHR to provide alerts in key clinical areas in real-time.

PERSONALIZED SERVICES FOR LONG TERM CARE:

We believe each resident deserves the highest level of personalized services and a care plan developed just for him or her. Most importantly, we put our residents at the center of care – everything we do revolves around them.

Interdisciplinary Team Conference

Manchester Manor uses a team approach in assessing and caring for our residents. This team approach is cultivated through the care planning process. The care planning team consists of:

• The family and resident
• Registered Nurses
• Certified Nursing Assistants
• Dietician
• Nursing Supervisor
• Social Worker
• Rehab Staff (if appropriate)

By working as a team, we gather the medical and social history to help understand where the resident is currently in his or her life process. A synopsis of the resident’s current status, concerns and goals are all addressed during these meetings. To ensure that the most appropriate care is delivered, we reassess our residents and their needs on a regular basis.

Nutrition & Dining

Manchester Manor has Registered Dietitians on staff that work with the other members of the interdisciplinary team to help enhance the nutritional care of each patient. The role of the dietitian includes the completion of a nutritional assessment and development of a plan of care that best suits the resident’s individual needs. This includes nutrition education for the resident and their family, monitoring appetite and weight changes, providing nutritional intervention for those with wounds and participating in resident care plan meetings.

Therapeutic Recreation

Through group and individual therapeutic recreation programs, we offer adaptations and modifications to ensure all residents benefit equally and achieve their goals. The primary goal is to assist our residents in maintaining and/or enhancing their optimal skills in physical, social, emotional, spiritual and cognitive functioning levels by providing meaningful therapeutic recreational activities.
